ResponsibilitiesThe Pharmacy Medication Reconciliation Technician, under the direct supervision of a pharmacist, will assist the care team through various phases of the patient’s care including but not limited to: 1) Gathering the best possible medication history that is then utilized by the care team for medication reconciliation, and 2) Assists with the provision of medications at the time of discharge from the hospital.
QualificationsEducation: High school diploma or equivalent required. Completion and maintenance of a Pharmacy Technician Certification Program required. Graduate of a pharmacy technician program preferred.
Experience: Minimum 2 years of experience as a pharmacy technician required; retail and/or inpatient hospital experience preferred. Experience applying customer service behaviors and communication skills required. Basic familiarity with computers and ke yboard skills and ability to navigate electronic systems applicable to job functions.
