Medical Pod Leader – Gloucester County, NJ

HarmonyCares , a national leader in home-based primary care, is seeking a Medical Pod Leader in the Gloucester County, NJ area. This is a unique opportunity to blend direct patient care with meaningful leadership—supporting a team of dedicated Advanced Practice Providers (APPs) and clinical care professionals.

What You'll Do

As the Medical Pod Leader , you will serve as the clinical lead for a designated team of providers, driving clinical quality, team collaboration, and performance. You’ll work closely with APPs, care team members, and administrative leadership in a dyad partnership with the Market Manager.

Key Responsibilities:

• Serve as collaborating and supervising physician for APPs and physicians within the pod
• Participate in ride-alongs and conduct high-risk patient visits as needed
• Provide mentorship and clinical development for APPs
• Lead care team huddles and ensure documented plans for high-risk patients
• Support clinical team members (SWs, CHWs, Nurse Navigators, Care Managers, etc.)
• Conduct routine chart reviews and provider panel assessments
• Review pod-level quality metrics and provider productivity; implement improvement plans
• Sign orders and documentation as needed, per state regulations
• Lead education around diagnosis accuracy, chronic condition management, and documentation
• Partner with HR on compliance, evaluations, and performance improvement
• Provide direct patient care for your own small panel and complex cases within the pod
• Collaborate with Regional Medical Directors and Market Office staff to drive pod performance

Qualifications

• MD or DO with current, unrestricted license
• Board-certified in Internal Medicine, Family Medicine, or Geriatrics (preferred)
• Previous experience leading or mentoring APPs strongly preferred
• Strong clinical judgment and understanding of quality measures
• Passion for working with medically complex, homebound patients
