Job Title: Primary Care Nurse Practitioner – AGNP / CNP | Wound Care Training Covered

Compensation: $105,000 to $120,000 annually

Benefits: Full Benefits Package (for full-time status)

Position Summary:
This position is ideal for a Nurse Practitioner (AGNP or CNP) who values flexibility, relationship-building, and clinical growth. Whether you’re wound care certified or not, we’ll cover the cost of certification so you can enhance your impact in this critical area of care.

Key Responsibilities:

• Deliver hands-on primary care and wound management in senior living settings
  
• Participate in chronic care management, palliative care planning, and clinical coordination
  
• Collaborate with interdisciplinary teams, family members, and facility staff
  
• Ensure timely documentation and professional communication
  

Qualifications:

• Licensed Nurse Practitioner in Minnesota (AGNP preferred)
  
• Background in geriatric care, internal medicine, or primary care
  
• Must have a valid driver’s license and reliable vehicle
  
• Interest in or experience with wound care and older adult health

• Navy: 1972 – Nurse Practitioner
  
• Air Force: 46Y3H – Nurse Practitioner
  
• Marines: NP – Nurse Practitioner
