Job Description: Family Medicine Physician

Department: Primary Care | Status: Full-Time Hours may vary including evening and weekends. On-call shared with other providers (1:4) | Location : Chicago, Illinois

Reports To: Clinical Director | Compensation: $190,000-220,000 (DOE)

Duties and Responsibilities:

The physician directs and oversees the provision of and provides quality care to those patients assigned to him/her consistent guidelines contained within the quality management, risk management, infection control, customer service and safety programs for MRHC.

  • Must be able to see patients from 0 to entire lifespan.
  • Assures the daily health center patient load is seen in a timely and clinically appropriate manner.
  • Assures all clinical activity occurring within the health center is accomplished according to acceptable guidelines for reimbursement.
  • Responds to calls within 20 minutes when on-call.
  • Adheres to approved MRHC treatment protocols.
  • Conducts patient education activities, including pre and post instruction exams and procedures, instructions related to illness and treatment such as diet and reinforcement of physician’s follow up instructions. Utilizes printed health education materials as appropriate and documents all instructions given. Considers an individual’s varying level of understanding and adjusts level of presentation to ensure the patient is knowledgeable of medical condition and plan of care.
  • Assures core team and staff properly carry out effective treatment and education.
  • Assures core team staff performs clinical tasks in accordance with accepted policies and procedures.
  • Participates in the orientation and training of staff as needed.
  • Ensures core team staff meets internal certification requirements for patient education and the administration of medication.
  • Assures follow-up records, as assigned, to determine conformance with accepted clinical protocols.
  • Ensures incident reports are completed for those occurrences defined as incidents in accordance with policy and procedure.
  • Ensures patients presenting with greater acuity than what is clinically appropriate within the health center are transferred appropriately.
  • Assures the patient’s medical record meets documentation standards.
  • Meets scheduled health center times and productivity standards.
  • Monitors and ensure the cost-effective utilization of clinical resources.
  • Work with Core Team and Case Management staff to provide for coordinated delivery of case management services.
  • Provides triage services for emergency walk-in patients, identifying those patients who can wait to receive care and those patients who require immediate medical attention. Schedules service for the patient within a time frame which meets the patient's medical needs.
  • Effectively demonstrates required competencies in the provision of educational, case management, triage and medication administration services.
  • Other duties as assigned.

Qualifications:

  • Doctor Osteopathy (D.O.) or a Doctor of Medicine (M.D.) Degree.
  • Completed Residency in Primary Care/Family Medicine is required and Board Certification in their primary care specialty is preferred.
  • Two (2) to three (3) years' work experience in the practice of primary care medicine in a community health center preferred.
  • Current Illinois registration and Advanced practice licensure.
  • Must have good verbal and written communication skills.
  • Must be able to respond appropriately in emergency situations, must demonstrate flexibility, leadership and teamwork ability.
  • ***Computer Skills A Must***
